All co-pays are due at the time of service. You will be rescheduled if you cannot pay your co-pay at the time of your appointment.

All self-pay patients will be required to pay a $100.00 fee for the first office visit. Any other charges incurred over and above that amount for that date of service and beyond, will be set up on a payment arrangement system.

If you have an insurance that has a deductible requirement, you will need to pay this amount prior to any surgery. This amount is due to us at least (3) three days before the procedure. If we do not receive payment prior to surgery, it will be cancelled with the option to reschedule.

Along with an in-house payment plan, we also offer CareCredit.
